  • Our services may be covered by your insurance provider if you have out-of-network care included in your insurance plan. In general, we participate in out-of-network coverage with commercial insurance plans. As a courtesy, we will file claims to your insurance for you so that you may get reimbursement according to your plan’s benefits. We do not participate in Medicare or Medicaid and do not submit to these plans for reimbursement. If you do not have out-of-network benefits or prefer to pay outside of your insurance plan, cash pay rates are available on inquiry. Please allow up to 2 business days for insurance verification after submission of this form.

  • Appointments, including your initial evaluation in the office, are usually 55 minutes. Longer sessions are available upon request.

  • Please wear clothing you feel comfortable moving in. We have some clothing (shorts, t-shirts) in the office if you require a change of clothing.

  • My office does not have a lobby. If the door is locked when you arrive, it means I am still with my previous client at the scheduled time of your appointment. I will open the door for you to enter the treatment area. If you require the restroom in the Union Square office, it is in the main hallway as you approach the office directly across from the second stairwell door. At your first appointment, we will discuss your care, do a full body assessment, and agree on a treatment plan. We will then begin our treatment session.

  • Home sessions are sometimes available upon request, subject to therapist availability.

  • I do not perform massage, and I am not a licensed massage therapist. As a physical therapist, I am highly trained in many modalities of manual therapy, which improve your body’s flexibility and mobility. Some types of manual therapy I provide include visceral manual therapy, neurological manual therapy, myofascial release, functional manual therapy, joint mobilizations, craniosacral therapy, and counterstain therapy. We do believe massage can be a useful adjunct to physical therapy, and we are happy to refer you to a massage therapist we work with if you would like to supplement your treatments with massage.
